NON-PAYMENT: If your charge is declined, you will be charged an additional $25 processing fee for each declined charge. FIRST MONTH BILLING: Your membership can begin on any day of the month but will be charged for the full month of the date of sign up. CONTRACT LENGTH: Your contract will be for 12 consecutive months beginning today. After the completion of 12 consecutive months, your contract will expire. At that point, you may enter into another contract. MEMBERSHIP CHANCE OF UPGRADE: You may change or upgrade your membership as long as the monthly fee does not decrease. You will need to fill out a new membership application. All pre-paid months from the original contract will be included in the new membership. NO TRANSFERS OR ASSIGNMENTS: Your membership is for you and cannot be shared between members. CONTRACT CANCELATION: This contract cannot be cancelled for any reason. If a contract must be cancelled for any reason, a cancellation fee of 50% the remaining months fees for the contact will be assessed. TREATMENTS: Each membership offers a set allocation of treatments each month. Treatments cannot be substituted. Unused treatments cannot be carried forward into subsequent months if unused. MISSING OR "NO-SHOW" APPOINTMENTS: If you are unable to keep a scheduled appointment, without 24 hours' notification (no-show) the appointment cannot be rebooked that month or carried forward to the following month. IF an appointment must be rescheduled due to a technicians non-availability (e.g. sickness), you may reschedule the appointment even if the appointment falls in a new billing month. AVAILABILITY OF APPOINTMENTS: We cannot be responsible for unavailability of appointments and we strongly advise members to pre-book well in advance.
